Need to factory reset LS-QVL

Have a Linkstation LS-QV12TL/R5 that originally had one bad HD.  During the process of replacing it, somehow it never worked.  Do not need the data, just need to know how to get the thing back to factory condition (RAID 5).  Now, I can't even access the drive.

Are there Bullalo service outlets around Indianapolis??  I've spent days trying to figure this out.
