Does anyone know how to reset this machine to factory defaults? We have an old one we are trying to set up and the instructions in the documentation does not work.

I put together a guide for making a USB recovery disk for these devices:
https://buffalonas.miraheze.org/wiki/Create_Recovery_Disk_for_Intel-based_Terastation
You can use that to get the device into EM mode, then use TSUpdater in debug mode to reformat the disks and set up the firmware:
https://buffalonas.miraheze.org/wiki/Enable_Debug_mode
If you'd prefer I also put together a project with tools for running modern Debian Linux on these devices instead of the stock firmware:
https://buffalonas.miraheze.org/wiki/Installing_Debian_on_Intel_Terastations

Support will be happy to provide you a reinstall SD card for TS-V at nominal cost. You'll need a reader.
