LS220de38a with error code i12 - how to fix?

Hi all,

This morning I encountered an error on my LS220de38a: I12. After browsing the forums I've activated the e-mail status report. This is what it says:

I have to clue what DISK error status 0 or 24 means. Anyone a guess how to fix this issue?

Disk2 is failing and should be replaced.
PM me for TFTP / Boot Images / Recovery files  LSRecovery.exe file.
