I know this is old but I thought I would contribute and at the same time possibly get some help.
I'm having a problem formatting my LinkStation Duo Pro (LS-WTGL/R1-V3). I'm using the TFTP bundle referenced in FAQ (3 of 5) and am able to successfully cold boot with it.
Using the firmware updater for this model I can't get past the first (format) phase. After a couple a minutes I get the same error as reported by the OP in this thread.
I tracked down acp commander (gui) and the original and was able to determine what -cb (clear boot) is doing.

I see I've got about 85% free space before update and after I get the ACP STATE error I see that usage is at 100%. I'm still working out what all is happening here between the emergency bootloader, partitions, mounts, etc, and the firmware updater so I really don't know yet if this is even related to my issue.
The two drives I'm using were working when I pulled them from some other hardware some time ago. I am considering using the LinkStation Duo (non Pro) to see if I can get them initialized properly as I was able to get it up and running recently using the same great KB data amassed here.
Any pointers would be appreciated.
